This is written on a slate when there is a disagreement on how a scene is photographed.
It may be on how an actor reads a line, how something is photographed or a concern
on stage direction. The reason it's written on the slate is that it provides
evidence to the fact that if the person protesting is correct, they should not
be held responsible if the scene fails to meet expectations.
